Job summary

P3 Health Partners, Inc. is seeking a Provider Operations Coordinator for our Medford location. This on-site role supports HEDIS quality gap closures, risk adjustment activities, and provider engagement across clinics.

You will coordinate outreach, track performance, and collaborate with internal teams to meet evolving requirements. The candidate should have 2+ years in healthcare, experience with EHRs and Microsoft Office, and the ability to travel within Jackson and Josephine Counties.

Qualifications

• High school diploma or GED required; related associate degree preferred.
• 2+ years healthcare experience required; clinical health care, managed care, or Healthplan experience preferred.
• Experience with EHRs and Microsoft Office (Word, Excel, Outlook, PowerPoint)preferred.
• Knowledge of CPT II codes preferred.
• CPR certification preferred.
• Medical terminology knowledge required.

Responsibilities

• Build relationships with affiliate groups and act as liaison between groups and P3 departments.
• Support quality gaps (HEDIS/HOS) and documentation via outreach, data audits, and education.
• Coordinate patient outreach and scheduling to meet patient needs.
• Coordinate care by working with various departments to manage high/rising risk patients.
• Assist in meeting preparation and participate in CSR/IDT/JOC meetings.
• Conduct data entry and maintain documentation for quality initiatives.
• Educate patients on preventative screenings and reasons for them.
• Possibly conduct in-home HEDIS gap closures for selected measures.
