AdventHealth in Denver is seeking an RN Navigator to guide patients through the care continuum, from initial contact to follow-up. You will coordinate with physicians and staff, schedule referrals, and monitor outcomes to improve program effectiveness. Strong educational and psychosocial support skills are essential for patient advocacy and family communication.

The role requires RN licensure and a Bachelor’s of Nursing, with 1+ year in acute care nursing. Bilingual English/Spanish preferred.

• RN license required.
• 1+ year in acute care nursing.

Responsabilités

• Provides nursing care and guidance from initial contact through care continuum.
• Guides patients on treatment plans and schedules follow-ups.
• Tracks outcomes and maintains records to measure program effectiveness.
• Develops patient education programs and tools.
• Provides psychosocial support and facilitates patient education on trials.
