The Program Manager III supports the vision of creating an innovative care model for patients during vulnerable times of care transitions. Work Location and Hours: A caregiver in this position works remotely between 7:00AM - 5:00PM, with local travel to the CCAC and Independence FHC locations as needed. We are seeking candidates who reside in the Greater NE OH only!

Key Responsibilities:

  1. Develop, implement, manage, and maintain operational systems and activities.
  2. Serve as a liaison with all levels of the organization and outside community.
  3. Manage multiple priorities and projects with competing deadlines.
  4. Allocate time to meet completion requirements.
  5. Serve as a coach and mentor for other positions in the department.
  6. Review and monitor adherence to Human Resources policies and corporate compliance procedures.
  7. Interpret KPIs for payor contracts and population health.
  8. Work with healthcare data to identify trends and monitor outcomes.

Minimum Qualifications:

  1. Bachelor's degree in business or related field or equivalent experience (Associate's degree with five years of experience, or Master's degree with two years of experience).
  2. Minimum of three years project experience in the assigned area, supporting clinical systems and/or data/financial analysis.

Preferred Qualifications:

  1. Master's Degree.
  2. Familiarity with health system navigation and the patient journey/experience.
  3. Knowledge of statistical and financial analysis, primary research, and business plan development.
  4. Knowledge of health IT systems like population health platforms.
  5. Familiarity with risk stratification, care coordination, and value-based care models.

Physical Requirements:

  • Ability to communicate and exchange accurate information.
  • Working in a stationary position for extended periods.
  • Working with physical records.
  • Operating a computer or other office equipment.
  • Travel throughout the hospital system in some locations.
  • Move up to 25 lbs in some locations.

Personal Protective Equipment:

  • Follow Standard Precautions using PPE as required for procedures.

Pay Range: $30.41 - $46.38 per hour. Actual compensation will be determined based on experience, education, and other factors. Benefits include healthcare, dental, vision, and retirement.
