The Program Manager III supports the vision of creating an innovative care model for patients during vulnerable times of care transitions. Work Location and Hours: A caregiver in this position works remotely between 7:00AM - 5:00PM, with local travel to the CCAC and Independence FHC locations as needed. We are seeking candidates who reside in the Greater NE OH only!

Pay Range: $30.41 - $46.38 per hour. Actual compensation will be determined based on experience, education, and other factors. Benefits include healthcare, dental, vision, and retirement.
