  • Familiarity with care management platforms, reporting systems, and healthcare data analytics preferred.
  • Experience working with external partners, including academic evaluators or state agencies preferred.
  • Bachelor’s degree in healthcare administration, public health, business, or related field or equivalent work experience
  • Project Management certification (PMP or equivalent) preferred but not required
  • Master’s degree preferred

Responsibilities

  • Lead Community Health Worker Pilot implementation across planning, execution, monitoring, and evaluation.
  • Develop and manage project plans, timelines, and milestones aligned to state requirements and program design.
  • Coordinate care management, behavioral health, pharmacy, provider, community partner, and external stakeholder teams.
  • Ensure staffing, workflows, training, and operational readiness for pilot launch and execution.
  • Monitor engagement, utilization, quality, and social determinants of health intervention metrics.
  • Identify and manage risks, issues, and dependencies and implement mitigation strategies.
  • Oversee internal and external reporting to state partners and evaluators.
  • Collaborate with external evaluators, including Rutgers Center for State Health Policy, on data collection, validation, and analysis.
  • Ensure compliance with Medicaid, DMAHS, and contractual requirements.
  • Facilitate governance through status meetings, stakeholder updates, and decision‑making forums.
  • Support workflow, documentation, and operational process development and refinement.
  • Partner with leadership on pilot scalability planning and long‑term sustainability.
