Job summary

Brilliant Corners seeks a Case Manager for Permanent Supportive Housing (PSH-PB) at the Gardena, CA project site. You will provide intensive case management to up to 20 clients, focusing on health, mental health, and housing stability amid a client-centered approach.

The role requires experience with homeless populations, a relevant degree, and a valid CA driver's license with a vehicle. You will coordinate housing placements, services, and crisis support within a multidisciplinary team.

Qualifications

• ,Bachelor's degree in Social Work, Psychology, Sociology or related field.
• Experience with homeless individuals and mental health/ intensive case management.
• Valid California driver’s license and a personal insured vehicle are required.

Responsibilities

• Accept, screen, and process referrals from HSH and partners
• Conduct outreach and engagement to locate referrals
• Conduct assessments and re-assessments to determine eligibility and needs
• Develop and implement individualized case management plans for housing stability
• Provide support through housing process including placement, leasing, and retention
• Provide tenant rights education, eviction prevention counseling, and advocacy
• Link clients to community resources and services
• Meet clients regularly in homes, on site, or in the field
• Provide crisis intervention and 24/7 crisis line access
• Teach life skills such as budgeting, scheduling, and safety
• Transport clients as needed and accompany to appointments
• Coordinate with onsite property management and case conferences
• Collaborate with partners and maintain client files and documentation
• Participate in staff meetings and agency events
