Qualifications

• Master's degree with 1 year, Bachelor's with 2 years, or Associate's with 4 years experience with persons with disabilities.
• Current PA driver's license with a good driving record.
• Clearances: PA criminal check, child abuse, and FBI.

Responsibilities

• Coordinate the referral, intake, and discharge process for program participants.
• Develop and monitor Individual Support Plans for participants.
• Provide counseling and resources for participants and families.
