About This Job

Minimum Qualifications
Five years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR
Six years of full-time work experience in any field plus two years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR
A bachelor’s degree plus two years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR
An advanced degree or a Juris Doctorate plus one year of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services.
Experience Substitution: Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field.

Job Duties and Other Information
Serves as a My Choice Transition Coordinator (TC) for the MCL project. These are work-from-home positions, domiciled to ensure statewide coverage. MCL is a program within LDH, operated jointly by the Office of Aging and Adult Services and the Office of Behavioral Health. The program aims to transition Medicaid recipients with Serious Mental Illness from nursing facilities to community settings, including assessment, support identification, and transition management.

Occupational Summary
The TC assesses, plans, coordinates, facilitates, and monitors transitions from nursing facilities to community settings, working with various teams and agencies. The TC reports to the Program Manager 1-B and supports policy development, community capacity building, and inter-agency collaboration.

Essential Duties
75% - Planning, implementing, and monitoring transitions, including assessments, developing transition plans, locating housing, coordinating with health and behavioral health services, and tracking cases.
10% - Developing relationships with local ombudsmen and community organizations to support transitions.
10% - Maintaining resource inventories and fostering community partnerships.
5% - Recommending policy revisions and participating in inter-agency activities.
