The Louisiana Department of Health seeks a My Choice Transition Coordinator to oversee the transition of individuals from nursing facilities to community settings. This role requires strong planning and assessment skills, a commitment to quality, and the ability to coordinate efforts among various stakeholders to facilitate successful transitions.

The TC is responsible for the assessing, planning, coordinating, facilitating, and monitoring the transition of individuals from nursing facilities to community settings using guidelines established by OAAS.The TC directly reports to the Program Manager 1-B for their assigned area. The incumbent will coordinate, support, and track the activities of transition teams consisting of OAAS Regional Office staff, Medicaid enrolled support coordination agencies, Medicaid managed care organizations (MMCOs), and Medicaid providers in planning and executing transitions. The TC will provide technical assistance, facilitate communication between the team and identify and address specific issues preventing individuals from transitioning. The incumbent will participate in policy development and planning to maintain and/or enhance community capacity to support transition targets. This will include combining state and local initiatives and resources to support participants to transition. Incumbent may also participate in activities that involve multiple administrative units within the Louisiana Department of Health and across multiple state agencies.
Incumbent must adhere to pertinent laws, rules, regulations, policies, and procedures of LDH, OAAS, Civil Service and appropriate state/federal licensing and governing bodies. Performs other duties as assigned for smooth operation of the agency.
