Job summary

MyPath in Sheboygan, WI is hiring a Service Coordinator (Program Manager) to lead multiple programs serving adults with behavioral/medical disabilities. You will work with guardians, clients, and staff, managing ISPs and field operations, and handling HR/payroll for the team.

The role requires field work with documentation time in the office; homecare experience is highly preferred, with a focus on coordination, safety, and client wellbeing.

Qualifications

• High School Diploma or equivalent; Bachelor’s degree preferred.
• 3 years direct care experience and 2 years supervisory experience preferred.
• Valid Wisconsin driver’s license with 3 years of acceptable driving record and a personal vehicle with insurance.

Responsibilities

• Interview and onboard new hires, including training on standard precautions.
• Develop and manage ISP for assigned clients across programs.
• Ensure services delivered as outlined in ISP; respond to emergencies incl. out-of-hours.
• Oversee household operations: maintenance, meals, housekeeping, vehicle maintenance, activity planning.
• Perform HR and payroll tasks for direct reports: interviews, orientations, training, performance monitoring.
• Manage client funds and valuables responsibly; maintain on-call responsibilities.
