Job summary

Socket.dev seeks an experienced Program Manager to oversee direct service provision and front desk operations for Seneca Heights Apartments. The role supervises 3 case management staff, an activities coordinator, and a front desk supervisor, and requires strong knowledge of housing first, harm reduction, and trauma-informed care.

The candidate will manage budgets, ensure data integrity in HMIS, and coordinate with county and SEPH for housing referrals.

Qualifications

• Master’s degree in human services required.
• At least five years’ experience with mental health and substance use treatment.
• At least five years’ experience working with families and individuals in a clinical environment.
• Minimum of two years’ supervisory experience.
• Knowledge of case management, housing first, harm reduction, and trauma-informed care.
• Knowledge of addiction, mental illness, and co-occurring populations.
• Ability to manage fiscal accountability and adhere to grants/contract requirements.
• Proficiency with word processing and spreadsheet software.
• Familiarity with HMIS/ServicePoint.
• Reliable car to transport clients as needed.

Description

General Description: The Program Manager oversees the direct service provision and front desk operations of Seneca Heights Apartments.

Essential Duties and Responsibilities:

• Provide direct supervision to 3 case management staff, 1 activities coordinator, and 1 front desk supervisor.
• Manage program budget for Seneca Heights, including timely submission of expense reports and vendor invoices.
• Track all unit vacancies and coordinate with property management staff to ensure a timely, thorough turnover of vacant units.
• Coordinate the delivery of items needed for clients to move into units (furniture, household items, etc.)
• Oversee the upkeep and safety of the facility by ensuring regular walkthroughs with property management, monitoring maintenance and cleanliness, and ensuring timely resolution of all identified issues.
• Ensure cohesive collaboration between case management and property management teams to support effective communication, timely resolution of client related issues and the overall safety and upkeep of the facility.”
• Submit program vacancies to the Coordinated Entry System/Services to End and Prevent Homelessness (SEPH).
• Attend Coordinated Entry System meetings to coordinate and track the referral process to ensure the timely transition of referred clients into housing.
• Assist referred clients with the application process, including the gathering of eligibility documentation, for submission to property management staff.
• Be available for on-call emergencies
• Provide bi-monthly board report information to the Senior Operations Officer
• Ensure the accuracy and completeness of all client data, including case notes, individual service plans, home visit reports, and budgets, in HMIS, Apricot 360, and all additional client databases.
• Monitor and submit contract deliverable data to county contract monitor and other stakeholders
• Represent programs at service provider meetings.
• Assist case management staff and property management staff to coordinate timely completion of rental recertifications
• Provide back-up case management as needed
• Coordinate with the Development Team to engage volunteers and donors to assist with client needs.
• Coordinate with external service providers to ensure continuity of care.
• Develop on-site programming for client enrichment and care.
• Ensure appropriate psychosocial groups and training are available on-site for residents
• Manage psychiatric and medical emergencies
• Other duties as assigned
