Job summary

You’ll build strong relationships with families and case managers while ensuring high-quality, person-centered care. This full-time role requires 1–2 years of managerial experience, a valid MN driver's license, and the ability to pass a DHS background check.

Qualifications

• Meet MN 245D requirements for program management.
• Bachelor’s or higher in relevant field with direct care experience.
• Alternative combinations with related training and years of experience are acceptable.
• Experience leading DSP teams and coordinating care plans is preferred.

Responsibilities

• Lead and support a team of DSPs delivering person-centered care in both hourly and 24-hour supported living services.
