Job summary

Sunrise Services, Inc. in Everett, WA is seeking a Program Coordinator for the Health Homes Program to connect clients with health and social services. You will work closely with a multidisciplinary team to coordinate care and support client goals.

Qualifications

  • Education: BA in Social Services or related field.
  • Two years supervisory/management experience required.
  • Washington state driver's license, acceptable driving record, and proof of insurance.

Responsibilities

  • Work as a member of a multidisciplinary team.
  • Provide administrative support and supervision to team members, including approval of timesheets, leave requests, supply requests, mileage and expense reporting, completion of performance reviews as scheduled, disciplinary incidents, and monitoring compliance of training requirements.
  • Screens referrals for eligibility and appropriateness. Assign accepted clients to appropriate team members.
  • Notifies referral sources of acceptance into the program or inform them why the client could not be accepted. Notify them of any delays in starting services and approximate timeframes. Maintain a waiting list, when needed.
  • Ensure that all clients have a Health Action Plan and oversee development and needed revisions to address clients’ changing needs.
  • Provide regular and supportive supervision to team, ensuring that they fully understand program objectives, case note documentation process, program requirements, best practices, and agency and program policies and procedures.
  • Monitors contract changes and negotiations with all signed contracts and CCOs.
  • Attends all contractual meetings as a representative of Sunrise Health Homes Program.
  • Facilitates all staff meetings and team lead meetings and completed one-on-one meetings with Care Coordinators for caseload reviews, progress checks, to include routine coaching and support to drive program divisional and agency goals and objectives.
  • Processes clients that are opting out of the Health Homes program.
  • Completion and oversight of audits and reviews with all contracts.
  • Manage due diligence completion to ensure accuracy and communicate changes with insurances.
  • Identifies and completion of reinitiated services for returning clients to contracted CCO’s.
  • Oversees all new hire interviews, candidate selection, and new hire training and orientation.
  • Manages all Release of Information and Records Requests.
  • Oversees and manages program office spaces where staff are located - in Everett, Mt Vernon, and Bellingham. Reports any maintenance problems.
  • Works in partnership with leaders of co-located programs to ensure effectively shared resources and spaces.
  • Actively markets and promotes awareness of the Health Homes program. Oversees team marketing and networking activities.
  • Effectively manages program budget - ensure effective tracking systems are in place.
  • Seeks consultation and feedback from department head, executive management, program staff, clients, family members, and residential providers.
  • Develops satisfaction surveys to be completed by clients, referral sources and residential providers. Collect, compile, and report data from returned surveys.
  • Develops appropriate short term and long-term objectives, strategies, standards, and processes for Health Homes operations with the goal of continuous measurable improvement.

About the Program

Health Homes is an outreach program which engages clients (including pediatrics) in the community with health and social services to best meet their needs. Health Home (HH) Services are intensive services that coordinate care across several domains. The purpose is to coordinate clinical and social service expertise for high risk clients with complex chronic conditions, mental health challenges, substance use disorder issues, and/or long term service needs and supports.

Duties & Responsibilities

Education/Experience

Education : B .A. of Social Services, or related field .

Experience: At least two years supervisory / management experience required .

Other Considerations :

Current valid Washington State driver's license, acceptable driving record, and proof of insurance.

Compensation
  • $ 34.62 - $40.38/hr
  • Wages are determined by applicable industry experience, education, and certifications/licensure.
  • Paid vacation - Earned at .04 per hour worked for hourly employees, and 6.67 per month for salary (equates to about 2 weeks per year for full-time employees)
  • Sick Leave - 1 hour per 40 hours worked (equates to about 52 hours for full-time)
  • Holiday pay, with 8 holidays observed
