Job summary

Sunrise Services, Inc. is seeking a Clinician to join our Extended Community Services and Specialized Dementia Care Programs in Washington.

The role focuses on coordinating care, developing tailored behavior support plans and providing crisis intervention within a collaborative, mission-driven team. The position requires a Master’s degree in counseling or related field (or equivalent), licensure as MHP/BCBA or supervised practice, and willingness to travel across Skagit and Snohomish counties

Qualifications

  • Education: Master’s degree or higher in counseling or social sciences, or Psychiatric Nurse; or bachelor’s with two years of dementia care experience.
  • Licensure: Must be certified as MHP, BCBA, or Agency Affiliated Counselor or work under supervision if not certified.

Responsibilities

  • Coordinate and facilitate care conference meetings to address challenging behaviors and community placement.
  • Create client-specific positive behavior support plans and monitor their use across settings.
  • Provide crisis intervention and consultation by phone and respond to after-hours requests.
  • Coordinate services with DSHS, doctors, hospitals, and community providers.

Job description

  • Base Pay $25.00 - $30.14 / Hour

Sunrise Services, Inc. is currently seeking a Clinician for our Extended Community Services Program & Specialized Dementia Care Program!

This includes travel in Skagit and Snohomish county.

Duties & Responsibilities:
  • Coordinate and facilitate care conference meetings to problem solve and address issues relating to challenging behaviors and community placement
  • Create effective client specific positive behavior support plans / Individual support plans to be utilized by clients and residential providers in community settings. Monitor use of the
    plans and provide trainings and consultation as needed to ensure effective use of the plan. Update and modify plans as needed.
  • Provide crisis intervention and consultation by phone as needed.
  • Respond to out-of-hours requests for crisis intervention and consultation as needed.
  • Coordinate services with community agencies such as DSHS, doctors, hospital staff, discharge planners, other MH Providers, courts, Day Services, Home Care agencies,
    Employment Support agencies, residential providers etc.
  • Seek consultation and feedback from clients, family members, agency staff and residential providers
  • Provide services and treatment in accordance with the evidence-based practice recovery model and document the same.
  • In coordination with community agencies and professionals, provide mental health services as appropriate including: direct clinical services such as intake, assessment, diagnosis,
    treatment, and crisis intervention; consultation/liaison with government and community agencies, hospitals, and other programs; community education, development, coordination
    and prevention/intervention programs; and program development and evaluation.
  • Consult existing clinical staff and medication management services. Provide timely and accurate compliance with all required and appropriate charting and
    coding as required by regulatory organizations such as Medicare, Medicaid, Home and Community Services, etc.
  • Comply with involuntary treatment admission (ITA) requirements as appropriate and/or coordinate least restrictive order (LRO) services if needed. Notify CDMHPs, write
    affidavits, and serve as a witness in court when involuntary treatment is indicated if needed establish and clarify involvement if an existing MH Provider is also working with client. indicated (at the end of 72 hours, 14 days, 90 days, etc.) if needed/appropriate.
  • Monitor compliance with LRO (Least Restrictive Order) as needed.
    • Deliver services as stated.
    • Seek continuance when indicated.
    • Complete all required documentation in a timely and thorough manner consistent with approved practices, policies and procedures.
  • Attend continuing education training as required by state regulations. Attend all training required by Sunrise.
  • Maintain all on-going licensing and certification requirements for this position and supply Sunrise proof of the same.
  • Monitors illness management and resource needs of assigned clients.
  • Develop consumer-focused, goal-oriented treatment plans, which address the specific needs of the consumer. Consult with existing MH providers when present to compliment
    and extend existing treatment plans.
  • Evaluate treatment effectiveness and revise care plans regularly.
  • Maintain consumer records in keeping with agency, county, and state guidelines.
  • Act as a backup for other team members.
  • Respond promptly to client needs.
  • Support and encourage recovery model.
  • Utilize clinical supervision for clarification of questions and concerns, support, understanding role expectations, feedback and accountability.
  • Develop expertise in solution focused and cognitive behavioral therapy and group process, provide this treatment, and share this knowledge with other staff.
  • Perform other duties as assigned.
  • In addition to the above, any other responsibilities appropriate to the position and not specifically listed in this job description or tasks that may be assigned on an as needed
    basis.
Requirements:

(Any Equivalent Combination of Knowledge, Skills, Abilities, Education, and Experience)

Education : Master’s degree or further advanced degree in counseling or one of the social sciences OR Psychiatric Nurse, OR a bachelor's degree in a related field and two years' experience working with adults with dementia or other neurocognitive disorders, and exceptional long-term care needs due to a combination of medical and behavioral needs. Clinicians who are not currently an MHP, BCBA or GMHS must work under the supervision of a MHP, BCBA, or GMHS.

Licensure/Certification : Clinicians must be certified as a Mental Health Professional (MHP), Board-Certified Behavior Analyst (BCBA), or Agency Affiliated Counselor, (Licensed, or Certified, Clinicians who are not currently an MHP, BCBA or GMHS must work under the supervision of a MHP, BCBA, or GMHS.)

Other Considerations :
  • DSHS background clearance required .
  • Current valid Washington State Driver’s License, acceptable driving record, and proof of insurance.
  • Skill using Microsoft Office products including Excel, Word and Outlook.
  • Strong verbal and written communication skills.
  • Flexible. Demonstrated ability to exercise sound judgment in challenging dynamic situations including behavioral issues.
  • Skill in reading, synthesizing, and acting upon multifaceted information.
  • Ability to write effective positive behavior support plans specific to client need
  • Ability to train and coach caregivers in the implementation of positive behavior
  • support and de-escalation.
  • Extensive knowledge of clinical issues pertaining to adult client populations, along with extensive understanding of the clinical process and intervention strategies
  • used to address these issues.
  • Relate effectively with clients, agency staff, other community programs, and the general public.
  • Work across agencies and disciplines, and as a part of a multidisciplinary therapeutic team.
Compensation:
  • $25 - $30.14/hr
  • All wages depend on applicable experience, license, and education.
Schedule:

Monday - Friday, Full-time

  • Paid vacation - Earned at .04 per hour worked for hourly employees, and 6.67 per month for salary (equates to about 2 weeks per year for full-time employees)
  • Sick Leave - 1 hour per 40 hours worked (equates to about 52 hours for full-time)
  • Holiday pay, with 8 holidays observed
