Job summary

Empowered Milestones, LLC is a San Jose-based adult Behavioral Day Program serving individuals with developmental and intellectual disabilities. We seek dedicated staff who enjoy helping others and will provide person-centered support to build skills and independence.

On-site roles include implementing individualized service plans, assisting with daily living skills, promoting client participation in meaningful activities, and ensuring safety.

Qualifications

• Experience working with clients with developmental disabilities and behavioral support needs.
• Strong communication and problem-solving skills.
• Able to follow individualized support plans and provide direct care.
• Reliable, patient, and committed to person-centered care.

Responsibilities

• Implement individualized service plans focused on social skills, communication, community integration, and safety awareness
• Assist with daily living skills, including hygiene and mobility support when necessary
• Promote independence and encourage client participation in meaningful activities
• Communicate regularly with supervisors and family members, as needed
• Ensure the safety and well-being of each individual at all times
• Assist with redirecting and managing behaviors

Our staff work on-site, providing person-centered support and helping individuals build skills, increase independence, and meaningfully engage in everyday life.

What you'll do:
