West Virginia University Health System is seeking a Population Health Program Manager to oversee the financial, operational, and human resources of assigned population health initiatives. You will partner with health system and community partners and align with Population Health leadership to ensure program continuity.

Ideal candidates have 3+ years in program management or population health, with experience managing grant-funded programs; PMP credential is preferred.

Formación

• Associate's degree in healthcare, business, public health, or related field plus 6 years healthcare experience or Bachelor’s in related field plus 4 years experience.

Responsabilidades

• Oversees daily operations of assigned program(s) to ensure consistent, efficient services aligned with goals.
• Monitors grant-funded program compliance: scope of work, deliverables, timelines, expenses, data collection, funder reporting.
• Prepares and submits required reports to funders, payer partners, and health system leadership.
• Ensures activities comply with federal, state, and payer regulations and health system policies; maintains audit-ready documentation.
• Establishes workflows for new programs, including patient identification, enrollment, service delivery, documentation, escalation, and completion.
• Tracks program performance measures (enrollment, engagement, utilization, quality, financial outcomes) and reports to Director.
• Integrates health system and community resources into care within the program.
• Develops referral pathways and handoff processes to support coordinated, non-duplicative patient care.
• Engages clinics, providers, and payer partners to identify eligible patients and enroll them.
• Educates clinic staff, providers, and partners on program services, eligibility, and referral processes.
• Represents the program in meetings, committees, and cross-departmental initiatives.
• Assists with recruitment, interviewing, and hiring of program staff; leads onboarding and competency validation.
• Maintains communication with managers, medical staff, and partner organizations to resolve problems and support quality care.
