Job summary

Clarity Care Inc. is hiring for a Program Lead to supervise group home operations and provide direct caregiving support.

You will lead meetings, coordinate staffing, and ensure documentation is up to date while maintaining a clean and welcoming home environment. The role requires at least an associate degree in a health care field (or higher), or a related bachelor’s degree with experience in working with individuals with disabilities, and a valid Wisconsin driver’s license.

Qualifications

  • Must have Bachelor’s degree in Education, Psychology, Social Work or related fields, or equivalent experience.
  • At least 21 years old with a valid WI driver’s license and clean driving record.
  • Must pass WI Caregiver Background Check and any state checks as required.

Responsibilities

  • Hands-on caregiving and direct support with activities of daily living.
  • Lead group home meetings; communicate changes to staff, consumers and HR.
  • Assist with hiring, retention, discipline and staffing adequacy.
  • Complete all required documentation for tasks assigned.
  • Maintain a clean, safe, welcoming group home; perform housekeeping and laundry.
  • Prepare and serve meals, considering dietary needs and preferences.
  • Support community activities and accompany members to events.

Provides direct care support to our members with disabilities by assisting with activities of daily living; bathing, grooming, toileting, and hygiene as outlined by the individual service plan. Be comfortable using mobility devices and practice safe transfer and ambulation techniques like in assisted living facilities.Leads group home meetings and communicate changes with the home, consumers, staff, and organization to ensure everyone is informed and updated.Collaborates with HR in hiring, retention, discipline, and termination processes to ensure qualified employees and adequate staffing.Completes all applicable documentation related to tasks assigned.Welcoming Home. Maintains a safe, sanitary and welcoming group home by performing housekeeping (vacuuming, emptying trash, cleaning bathroom, dusting, bed making and changing linens); and laundry.Prepares and Serves Meals by following general nutritional guidelines, menus, individual dietary restrictions, needs and preferences to ensure consumer health.Community Activities.
