Job summary

Clarity Care in Wisconsin is seeking a Program Lead to manage and supervise staff and day-to-day operations in our group homes. You will oversee community involvement, care delivery, regulatory compliance, and staff development while ensuring residents receive respectful, person-centered support.

The role requires hands-on caregiving, ability to build a team, and a commitment to empowering people with varying abilities.

Qualifications

• Associates degree or higher in health care or related field (or a bachelor’s in a non-health field with 1 year direct care experience).
• Willingness to supervise others and lead group home operations.
• Strong people skills and community involvement.
• Valid WI driver’s license, auto insurance, and ability to meet driving standards.

Responsibilities

• Hands-on caregiving: assist with activities of daily living per individual service plan.
• Leads group home meetings and communicates changes to home, consumers, staff and organization.
• Collaborates with HR on hiring, retention, discipline, and staffing considerations.
• Completes all applicable documentation related to tasks assigned.
• Maintains a safe, sanitary and welcoming group home (housekeeping, laundry).
• Prepares and serves meals following dietary guidelines and individual needs.
• Supports community activities and assists members during social/recreational activities.

Provides direct care support to our members with disabilities by assisting with activities of daily living; bathing, grooming, toileting, and hygiene as outlined by the individual service plan. Be comfortable using mobility devices and practice safe transfer and ambulation techniques like in assisted living facilities.Leads group home meetings and communicate changes with the home, consumers, staff, and organization to ensure everyone is informed and updated.Collaborates with HR in hiring, retention, discipline, and termination processes to ensure qualified employees and adequate staffing.Completes all applicable documentation related to tasks assigned.Welcoming Home. Maintains a safe, sanitary and welcoming group home by performing housekeeping (vacuuming, emptying trash, cleaning bathroom, dusting, bed making and changing linens); and laundry.Prepares and Serves Meals by following general nutritional guidelines, menus, individual dietary restrictions, needs and preferences to ensure consumer health.Community Activities.
