Job summary

Clarity Care in Wisconsin seeks a Program Lead to supervise staff and manage daily group home operations with a focus on safety and quality. You will provide direct care to members, lead meetings, and collaborate with HR on staffing while ensuring documentation is complete and accurate.

Qualifications

• Associate degree or higher in a health care field or related experience.
• One year experience working with individuals with developmental disabilities or similar settings.
• Willingness to meet WI caregiver background checks and driving standards.

Responsibilities

• Hands-on caregiving, assisting with activities of daily living per service plan.
• Lead group home meetings and relay updates to staff and consumers.
• Collaborate with HR on hiring, retention, and staffing decisions.
• Document tasks thoroughly and maintain a clean, welcoming home.
• Prepare meals per nutritional guidelines and assist with activities.
• Support community activities and accompanying members during events.

Provides direct care support to our members with disabilities by assisting with activities of daily living; bathing, grooming, toileting, and hygiene as outlined by the individual service plan. Be comfortable using mobility devices and practice safe transfer and ambulation techniques like in assisted living facilities.Leads group home meetings and communicate changes with the home, consumers, staff, and organization to ensure everyone is informed and updated.Collaborates with HR in hiring, retention, discipline, and termination processes to ensure qualified employees and adequate staffing.Completes all applicable documentation related to tasks assigned.Welcoming Home. Maintains a safe, sanitary and welcoming group home by performing housekeeping (vacuuming, emptying trash, cleaning bathroom, dusting, bed making and changing linens); and laundry.Prepares and Serves Meals by following general nutritional guidelines, menus, individual dietary restrictions, needs and preferences to ensure consumer health.Community Activities.
