Job summary

Supportive Concepts for Families in Berks County is seeking an IDD- Program Lead to guide direct support operations across residential and community-based settings. This full-time role emphasizes training, documentation, and compliance, with daily notes, ISP alignment, and collaboration with leadership.

You will mentor coordinators, conduct monthly inspections, and ensure person-centered practices while supporting individuals to live meaningful, independent lives.

Qualifications

• Associates Degree or 60 credit hours with 4 years of direct IDD and Behavioral Health experience.
• Bachelor's Degree with 2 years direct IDD and Behavioral Health experience.
• Excellent communication skills; written and verbal.
• Compliance with residential setting regulations 6400.

Responsibilities

• Provide training and development for new and existing Assist Program Coordinators and the Program Coordinator Team Members.
• Maintain on-call program notes, daily case notes, and goal documentation.
• Attend all required scheduled meetings and work in conjunction with leadership.
• Mentor and provide recommendations for improvements.
• Conduct monthly site inspections and document for all licensed and unlicensed sites.
• Manage incidents as part of incident management.

What You'll Do

• As the Program Lead you will;

Qualifications Required

• Associates Degree or sixty (60) credit hours and four (4) years experience working directly with people with IDD and Behavioral Health
• Bachelors Degree and two (2) years experience working directly with people with IDD, and Behavioral Health Diagnosis
• Excellent communication skills
• Excellent writing, documentation, financial, and math skills
• Excellent organization skills, detail oriented
• Team Player
• Compliance with residential setting regulations 6400.

Schedule

• Full-Time
• Occasional travel to program locations and meetings
• Flexible scheduling as needed to support ISP meetings and stakeholder participation

Physical Requirements

• Individual must be able to meet physical requirements of the job: sitting at a meeting table or desk, reaching, lifting, bending, standing, stooping, twisting, climbing steps, driving a vehicle, ability to lift, push, and or pull up to thirty (30) pounds.
• Ability to travel to community, residential, and program settings as needed
• Ability to perform essential job functions with or without reasonable accommodation
