Job summary

S:US is seeking a Program Director in New York to oversee case management services for residents with mental health diagnoses, HIV/AIDS, and homelessness. You will supervise staff, develop client service plans, and coordinate with clinical teams to ensure quality care.

The role requires 3–5 years of supervisory experience, a Bachelor’s degree (Masters preferred), and experience with HIV/AIDS and mental health populations. Bilingual English/Spanish is a plus.

Qualifications

• Bachelor's Degree required; Master's degree preferred.
• Minimum of 3-5 years of supervisory experience.
• Experience with HIV/AIDS population and mental health.

Responsibilities

• Provide supervision of all program staff with regards to Case Management
• Develop and implement individual client service plans with clinical staff and clients
• Ensure contracts reflect service goals and support counselors' intervention activities
• Coordinate Annual Functional Assessment Reviews and report recommendations
• Maintain connections with other service agencies and client placements
• Coordinate with Intake to orient and transition new residents into the program
• Participate in discharge planning and provide emergency coverage as needed
• Represent the agency with community groups and partners
• Prepare reports for leadership and propose policy or program changes
• Conduct supervisory meetings and maintain staff meeting documentation
• Work flexible shifts including weekends as assigned
• Perform other duties as assigned
