The Bridge offers supportive housing and behavioral health services to vulnerable New Yorkers facing behavioral health and substance use challenges. As a recognized leader in developing and operating supportive housing and community-based programs, The Bridge has 70 years of experience helping New Yorkers with mental illness, including those affected by homelessness, incarceration, and institutionalization.

Scope of Position: We're seeking a Director to lead the New York State Office of Mental Health Critical Time Intervention team. This role involves overseeing a multidisciplinary team including an Assistant Director, Master’s level Clinicians, Care Managers, a Registered Nurse, and a Peer Specialist. The Director will initially ensure successful program implementation and subsequently manage day-to-day operations. Essential qualities include a management style that values member choice, harm reduction, non-coercion, flexibility, and a person-centered approach. Building and maintaining collaborative relationships with community partners, especially hospitals and behavioral healthcare providers, is crucial. This position offers a chance to demonstrate leadership in a dynamic environment, contributing to transformative community healthcare initiatives in NYC. Field-based work, periodic on-call coverage, and flexible hours are required, reflecting our commitment to making lasting positive impacts in the lives of vulnerable New Yorkers.

Essential Position Functions:

  • In partnership with the State Office of Mental Health oversee the creation of a multi-disciplinary CTI community support team.
  • On formation of the team, management of the day-to-day operations in accordance with internal and external regulatory standards and guidelines.
  • Coordinate program outreach during an inpatient hospital admission or emergency department visit to both engage in relationship building with individuals served and partner in discharge and aftercare planning with hospital staff.
  • Develop meaningful and lasting partnerships with key community stakeholders including local hospitals, criminal justice system, shelter operators and housing providers.
  • Facilitate case reviews on ‘members at risk’ with community partners and document recommendations.
  • Oversee and or conduct internal audits/case record reviews on a regular basis to measure compliance to regulatory/contract standards, and report aggregated findings and recommendations to the CTI Hub QPM Department on gaps in care or areas for continued improvement.
  • Assist with the identification, development, implementation, and completion of quality improvement projects that address barriers to care.
  • Ensures that internal and external regulatory agency standards and requirements are met.
  • Manage recruitment of staff, as well as address staff performance-related issues in accordance with salient policies and procedures specific to The Bridge.
  • Provide regular individual and group supervision to a multi-disciplinary team and orient new team members to policy and procedures as well as provide ongoing training and mentorship.
  • Coordinate weekly staff schedules to ensure adequate staff coverage.
  • Manage CTI Team's on-call requirements ensuring that 24/7 crisis response is available.
  • Work with CTI HUB’s Single Point of Access department to identify eligible participants and reporting on enrollment outcomes and staff assignment.
  • Work with all stakeholders (members, network, families, and government staff) in a caring and respectful manner, and with due understanding of and consideration for cultural .
  • Provide services in a strength-based, recovery oriented, trauma informed and culturally sensitive manner.

Requirements:

  • Licensed Clinical Social Worker or Licensed Mental Health Counselor.
  • A minimum of 3 to 5 years post-masters work experience in the behavioral health and criminal justice field with progressively increasing responsibilities, preferably with the target population. Previous management and supervisory experience .
  • Knowledge of treatment, rehabilitation, and community support programs as they relate to recipient/, families, and staff.
  • Ability to develop, evaluate, implement, and modify treatment intervention to meet the needs of individual recipients.
  • Experience managing and supervising program staff in a community mental health setting.
  • Well-developed clinical competence in evidence-based and recovery-oriented practices for high-risk, high-needs individuals in behavioral health settings.
  • Ability to prepare accurate and timely reports.
  • Proficient in electronic health records, preferably Foothold Technology AWARDS.
  • Fluency in Spanish .
