Job summary

Liberty Resources, Inc. in Syracuse, NY seeks a Program Director to oversee the Short-Term Crisis Residence Program, guiding staff to deliver peer support, wellness coaching, and relapse prevention planning within a safe, home-like environment.

The role coordinates care with clinical providers, manages day-to-day operations, ensures compliance with HCBS regulations, and partners with managed care to authorize services, while maintaining a commitment to diversity and exemplary client outcomes.

Qualifications

• Must have three to five years of management experience in a social service or related setting.
• Experience with Peer/Recovery services preferred.
• Qualified Health Professional listed as a Licensed social worker, alcoholism counselor, mental health counselor or rehabilitation counselor preferred.

Responsibilities

• Coordinate the day-to-day activities associated with managing the Short Term Crisis Respite program.
• Supervise assigned staff in teaching individuals to view crisis differently and utilize wellness tools to prevent future crises or reduce intensity.
• Ensure a friendly, safe and supportive home-like environment.
• Maintain collaboration with clinical treatment providers and care coordinators to determine continuation of necessary care and modify Plans of Care as needed.
• Establish relationships with Managed Care Companies and obtain authorization for services as needed.
• Oversee the Plan of Care and HCBS regulations including assessments and length of stay limits.
• Provide staff training and support to meet performance expectations.
