Job summary

Fraser seeks a Program Coordinator for its Community Living team in Minnesota. In this leadership role, you will supervise staff, support individuals, and ensure services align with plans and 245D regulations. You’ll train staff and maintain compliant documentation across the program.

The position oversees one Supportive Living apartment setting and requires coordination with families and healthcare partners, plus on-call coverage. Frasers mission drives impactful care.

Qualifications

• 2+ years of professional experience working with individuals with mental health needs.
• 1 year of supervisory or program coordination experience preferred.
• Valid driver's license.
• Must meet 245D qualification requirements: options include high school diploma + 4 years direct care, associate degree + 2 years direct care, or bachelor’s degree + 1 year direct care.

Responsibilities

• Provide leadership, supervision, coaching, and support to Community Living staff.
• Ensure individuals receive quality services per plans, Fraser policies, and 245D regulations.
• Coordinate intakes, discharges, and services with families and healthcare providers.
• Develop and provide staff training and support professional growth.
• Resolve day-to-day questions and conflicts for staff.
• Ensure documentation, billing, payroll, and records are completed accurately on time.
• Maintain HIPAA, 245D, and Fraser policies compliance.
• Respond to urgent needs and participate in on-call rotation.
