Job summary

Benchmark Human Services is seeking a Program Coordinator for the Supported Living Program in Lima, OH. You will develop and implement the Individual Service Plan, coordinate services across providers, and support client participation in community life.

This role includes on-call coverage and involvement in hiring and training of staff. Qualifications include a Bachelor's degree in human services and relevant experience; CPR/First Aid are required.

• Valid CPR and First Aid.

Responsibilities

• Develop and implement the Individual Service Plan and assist in development of the Plan of Care for each person served.
• Coordinate the delivery of all services rendered by Benchmark and other providers.
• Assist people served in expanding their opportunities to participate in community life.
• Provide on-call direct support and emergency coverage.
• Participate in hiring, training and supervision of Team Leaders, Supported Living Managers, and Direct Support Professionals.
