Summary
- The Program Coordinator, RN provides clinical oversight and direct patient navigation of the specific patient population through the Department. Under the direction of the Department Clinical Leadership and Division Section Chief/Departmental Chair, the incumbent will oversee the development, implementation, and day-to-day administrative functions of the program, ensuring coordination throughout the patient continuum in collaboration with associated care teams. This includes multimodality dynamic scheduling, triage services, reporting of key metrics, and coordination of care across multiple specialties and services.
Responsibilities
- Ensure well-coordinated, timely, compassionate, and multidisciplinary care by functioning in an organized and time-conscious manner. Coordinate patient tests, procedures, appointments, and treatments across healthcare disciplines.
- Manage an office-based patient communication system to improve access to care (phone, email, Penn Chart, MPM).
- Identify gaps in the program pathway and evolving needs; initiate solutions and interventions.
- Primary contact with vendors including Medtronic, Edwards, Shockwave, and M2s.
- Review referring provider lists and collaborate with Penn marketing and outreach to detect patterns.
- Build and maintain relationships with referring providers for seamless patient flow and communication.
- Be knowledgeable of insurance matters and collaborate with the business office and financial services regarding patient issues.
- Lead multidisciplinary care conferences with internal and external stakeholders.
- Partner with practice leadership to improve access and volume metrics, including scheduling, registration, and patient experience.
- Continuously assess, plan, implement, and evaluate patient care.
- Promote programmatic research goals.
- Report access and volume metrics to stakeholders.
- Collaborate with clinical providers and care teams to develop and maintain navigational reporting structures for patient progress tracking.
- Supervise workflow of Administrative Assistants and coordinate testing and visits.
- Assist patients in overcoming barriers to care such as transportation, financial counseling, and referrals.
- Serve as a patient-facing liaison between the program and referring providers.
- Contribute to strategic development and workflow improvements in collaboration with leadership.
Credentials
- Registered Nurse - PA (Required)
- Basic Cardiac Life Support (Required)
Education or Equivalent Experience
- Bachelor of Arts or Science (Required)
- 5+ years nursing and/or healthcare and/or business experience
- Master of Arts or Science (Preferred)
