Midwest Health Services, Inc. is seeking a Day Services Program Coordinator to support individuals with ID/DD in home and community settings. You will collaborate with a multidisciplinary team to implement service plans and advocate for client rights, helping people live more independently.

The role emphasizes community integration, teamwork, and effective communication with guardians and families to empower those served.

Your Responsibilities Will Include, but are not limited to (full job description available at interview):

• Works in a collaborative team process supporting individuals with ID/DD

• Intellectual/Developmental Disabilities field as direct support/management
• Long Term Care – Assisted Living, Nursing Home
• Home Health Care
• Settings requiring teamwork – team leader, crew leaders
