The Fund for Public Health in New York City (FPHNYC) is a 501(c)3 non-profit organization dedicated to improving the health and well-being of all New Yorkers. In partnership with the NYC Department of Health and Mental Hygiene (DOHMH), FPHNYC incubates innovative public health initiatives to promote community health across the city. It fosters partnerships between government and the private sector to develop, test, and implement new projects, accelerating successful pilot programs and supporting rapid deployment to meet public health needs.
The Bureau of Equitable Health Systems (BEHS), part of the Division for the Center for Health Equity and Community Wellness at DOHMH, is located at 42-09 28th Street, Long Island City, NY. This division aims to strengthen the Department’s capacity to partner with NYC's healthcare system, engaging providers and organizations to implement evidence-based strategies, support planning, advance policies to close racial health disparities, and connect healthcare with social support services. The Community Healthcare Support Team (CHST) within BEHS manages NYC REACH, aiding providers in adopting new healthcare models, participating in payment reforms, and linking patients to community resources. BEHS seeks a Program Coordinator to support program staff and partners in implementing best practices for quality improvement initiatives, particularly within the Cardiovascular Health Innovation Program (CHIP), a CDC-funded project targeting health disparities in cardiovascular disease in high-risk NYC neighborhoods. Under the guidance of the Programs and Administration Manager and CHST leadership, the Coordinator will track grant and team deliverables, organize trainings and meetings, create agendas, disseminate materials, and ensure follow-up on action items. The role involves coordinating efforts across NYC REACH and BEHS to align with project and organizational goals.
