Job summary

Prairie Community Services, Inc. in Starbuck, MN seeks a Program Coordinator for Shorewood Home to oversee 24-hour operation, supervise staff, and ensure person-centered programming and regulatory compliance. Starting salary is $58,240 with strong benefits.

The role includes developing services, coordinating daily operations, maintaining records, and training staff to deliver quality care for individuals with developmental disabilities and mental health needs.

Qualifications

• Must be 18 years of age or as dictated by licensure.
• Must pass DHS background clearance.
• Must have valid driver’s license and meet company driving criteria.
• Must be able to read, write, communicate and understand the English language while conducting company business.
• Experience working with individuals with a mental health diagnosis.
• Experience in the field as a nurse, a two-year associate degree and/or experience as a direct care worker preferred.

Responsibilities

• Develop, implement, evaluate, update, and train on each person’s services, supports, and outcomes.
• Ensure needs and program expectations of persons served are met 24 hours/day.
• Follow recipes, menus and diets as ordered when preparing and serving meals/snacks.
• Ensure medical treatments and medications are ordered, secured, stored, documented, and given as medically ordered.
• Communicate professionally with people we serve and their family members, outside services and providers, all PCS staff, and the public.
• Develop and maintain staff schedules including approving/denying staff requests for time off.
• Ensure documentation of daily program information.
• Attend and/or lead all in-services, develop and maintain staff schedules complete employee performance reviews, hire discipline and terminate staff.
