Job summary

Qualifications

• A Bachelor's degree and one year work experience; or high school diploma (or equivalent) and 2 to 4 years work experience

Responsibilities

• Support RN Case Managers and Social Workers in collaboration with the interdisciplinary healthcare team across the continuum.
• Coordinate payer communication with external stakeholders and internal departments.
• Facilitate transition plan implementation and ensure timely delivery of patient documentation.
• Contribute to case management departmental functions and documentation.
