Job summary

Centerstone is seeking a dedicated Program Coordinator in Joplin, MO to oversee ISP development and ensure quality of care within developmental disabilities programs. You will coordinate team efforts, monitor documentation, and promote wellbeing through community and residential supports.

The role requires a bachelor’s degree and relevant experience; a valid driver’s license and strong communication are essential.

Qualifications

  • Bachelor’s degree from an accredited college or university.
  • Relevant experience in Developmental Disabilities can substitute year-for-year for a degree.
  • Current driver’s license and valid auto insurance.
  • Ability to communicate clearly, read and write effectively.
  • Ability to assist with physical transfers as needed.

Responsibilities

  • Coordinate and participate in development of Individual Support Plans (ISPs).
  • Ensure compliance with policies and protect health information per HIPAA training.
  • Supervise and evaluate team members to promote quality of care.
  • Conduct quarterly Records and Therapy Reviews for documentation accuracy.
  • Visit individuals in residential settings to monitor health and safety.
