Job summary

Community Counseling Center is seeking a non-clinical Program Coordinator to lead Local Hub 25, serving Bollinger, Cape Girardeau, and Perry Counties in Missouri. You will coordinate local Hub programs, work with hospitals, FQHCs, and community partners, and manage navigator workflows to improve access to behavioral health and social services.

The role focuses on operational leadership, referral pathways, and performance monitoring, with travel across the three-county hub.

Qualifications

  • Associate degree or an equivalent combination of education and relevant experience; bachelor's degree in public health, healthcare administration, social work, health informatics, or a related field is a plus.

Responsibilities

  • Serve as a primary spokesperson for the Hub, building relationships with healthcare providers and community partners to identify local needs and develop strategies to improve access and coordination.
  • Coordinate implementation of local Hub programs, statewide initiatives, including access expansion, budgets, timelines, and coordination among HUB members and delivery partners.
  • Oversee RHTP Navigator utilization and workflows, including closed-loop referrals, outreach activities, documentation practices, and alignment with Hub priorities.
  • Support Hub Leadership Board activities, including meeting coordination, agenda development, documentation of decisions, and follow-up on action items.
  • Coordinate onboarding and participation expectations for hospitals, FQHCs/RHCs, behavioral health providers, EMS, pharmacies, public health agencies, schools, and community organizations.
  • Develop and improve referral pathways, communication processes, escalation protocols, and operational workflows across Hub partners.
  • Monitor Hub progress and performance and identify risks, service gaps, or areas needing additional support.
  • Track and report operational, implementation, referral, and performance data using the CIE, dashboards, and other approved systems.
  • Use data and feedback from partners to identify opportunities for improvement and strengthen Hub operations.
  • Work with Hub partners and regional/state partners to address operational barriers and improve coordination

Education

Associate degree
Bachelor's degree in public health, healthcare administration, social work, health informatics or related field (plus)

Job description

Location: Bollinger, Cape Girardeau, and Perry Counties
Position: Full-Time
Schedule: Monday-Friday, 8:00 a.m.-4:30 p.m. (times may vary; occasional evening or weekend outreach may be required)
Travel: Frequent travel throughout the three-county Hub area which includes Bollinger, Cape, and Perry based on program needs

Salary Range: $55,000.00 - $60,000.00

The RHTP Program Coordinator Role

Community Counseling Center (CCC) is seeking an RHTP Program Coordinator to serve as the operational lead for Local Hub 25, serving Bollinger, Cape Girardeau, and Perry Counties.

Through Missouri's Transformation of Rural Community Health Care (ToRCH Care) model, the Program Coordinator brings together healthcare providers and community partners to improve access to and coordination of healthcare, behavioral health, and social services throughout the Hub area.

You’ll work closely with hospitals, FQHCs, Rural Health Clinics, behavioral health providers, pharmacies, EMS, public health agencies, schools, and community-based organizations to strengthen partnerships and coordinate local programs.

This position also oversees RHTP Navigator workflows, supports Hub Leadership Board activities, monitors program performance, and helps develop effective referral and care coordination processes across participating organizations.

This is a non-clinical position employed by CCC as the designated Hub Anchor organization. The Program Coordinator does not diagnose, prescribe, or provide licensed clinical treatment.

What You’ll Do

  • Track and report operational, implementation, referral, and performance data using the Community Information Exchange (CIE), dashboards, and other approved systems
  • Use data and feedback from partners to identify opportunities for improvement and strengthen Hub operations
  • Work with Hub partners and regional and state partners to address operational barriers and improve coordination
Minimum Requirements
  • Associate degree or an equivalent combination of education and relevant experience; bachelor's degree in public health, healthcare administration, social work, health informatics, or a related field is a plus.
  • 1-3 years of experience in healthcare operations, community health, care coordination, public health, quality improvement, or a related field
  • Previous experience in community health, care coordination, case management, patient navigation, Medicaid care management, behavioral health support, public health outreach, social services, or a related setting
  • Experience coordinating projects or initiatives involving healthcare providers, public agencies, and/or community organizations
  • Must hold or obtain by start date, MO Class E Driver's License (Class D for IL Residents), and proof of continuous auto insurance. Must also meet CCC's Motor Vehicle Record standards policy and have reliable transportation.
