Job summary

The Relatives is seeking a Program Case Management Supervisor to lead case management efforts for transitioning young adults. You will set direction, mentor staff, and ensure documentation standards while managing a small client caseload.

Responsibilities include TIP Model training, coaching teammates, and driving KPI results to improve program outcomes. Valid driver’s license and supervisory experience are required.

Qualifications

• Bachelor’s degree in a human services field with 1 year of related experience, or a bachelor’s degree in any field with 2 years of related experience.
• Supervisory experience preferred. Extensive supervisory experience may substitute for education.
• Valid NC Driver's License.

Responsibilities

• Set case management direction aligned with organizational goals and ensure compliance with documentation standards.
• Mentor case managers and provide TIP Model training.
• Manage a caseload of 7–10 clients personally.
• Promote ongoing team development through coaching and feedback.
• Measure and achieve program objectives and KPIs to drive outcomes for transitioning young adults.
