Union Settlement in East Harlem is seeking a Part-Time Program Assistant for the Aging, Health & Wellness Services department. The role supports the Older Adult Center Director by organizing daily activities and providing case assistance under NYC Aging contracts.

Responsibilities include intake/reassessment, benefits eligibility guidance, outreach to clients, and referrals to Medicare/Medicaid, Social Security, housing, and food programs.

Qualifications

  • Bachelor’s degree or 60+ credits with four years of aging population experience.
  • Bilingual English/Spanish required.
  • Strong verbal, written, clerical, organizational and problem-solving skills.
  • Ability to multitask in a fast-paced, changing environment.
  • Excellent computer and interpersonal skills.

Responsibilities

  • Identify, plan, and lead one center daily activity.
  • Conduct intake and reassess eligibility for benefits and entitlements.
  • Perform outreach to absent clients to check wellbeing and satisfaction.
  • Provide information/referrals for Medicare/Medicaid, Social Security, housing, Food Stamps, Meals on Wheels, energy assistance.
  • Maintain case records and follow-up on entitlements; refer to appropriate resources.

Position Summary:The Program Assistantis responsible forassistingthe Older Adult Center Directorbyorganizing and providing at least one activity daily and providing caseassistanceservices required by the NYC Agingcontract to senior participants daily.

Responsibilities
  • In consultation with the Older AdultCenterDirector,identify, plan, and lead one activity in the center daily.
  • Conduct intake and re-assessment for Older AdultCenter participants. Assess participants’ eligibility for benefits andentitlementsunder various programs for the aging.
  • Conduct outreach (phone calls, etc.) to all clients who are absent from the center for extended periods to determine the reason for absences, as well as to check on their well-being and satisfaction with the Senior Center.
  • Provide information and referralsorassistanceto the following but not limited to:Medicare and Medicaid eligibility and enrollment
  • Social Security eligibility and benefits
  • Housing services
  • Food Stamps
  • TheMeals on Wheels program
  • Foradditionalbenefits and entitlements
  • Energy Share and other utilityassistanceprograms
  • Provideparticipants with but not limited to:Generalassistancetoenroll in programs.
  • Translation services
  • Assistancefor correspondence, understanding and paying bills, and making appointments
  • Follow-up caseassistancerelated to entitlement and benefits, as needed, andmaintain,complete andaccuratecase records.
  • Refer clients who need case management or other in-depth services to appropriate resources.
  • Advocate on behalf of seniors with social service and governmentagencies, andmediate as needed with those agencies.
  • Attendappropriate trainingprograms as requested by supervisor.
  • Compile statistics, prepare reports, keep attendance, and other required records.
  • Additionalduties as assigned by supervisor.
Qualifications
  • Bachelor’s Degree in Social Work, Human Services, or related field with experience working with aging populations preferred; or 60+ college credits and four yearsdemonstratedexperience working with aging populations.
  • Bilingual (English/Spanish)required.
  • Strong verbal, written, clerical, organizational andproblem solvingskills.
  • Demonstrated ability to perform multiple tasks effectively in a fast paced, challenging and constantly changing environment.
  • Excellent computer skills.
  • Excellent interpersonal skills.
