About the Role
Family First At Home is looking for an experienced social worker who enjoys meeting clients where they are—often in their own homes—and helping families work through complicated healthcare and caregiving decisions.
This is a community-based role that combines psychosocial assessment, resource navigation, advocacy, counseling, and care coordination. Our Professional Care Managers support older adults and other clients living with chronic, acute, or terminal illnesses.
You will manage a caseload, develop care plans, connect families with appropriate services, and provide steady guidance during transitions or crises. The work is independent, but you will have a team available for consultation and collaboration.

Regular travel throughout Pierce and King Counties is part of this position. Business mileage incurred while traveling between work locations or visiting clients and referral partners will be reimbursed in accordance with the company’s mileage reimbursement policy.
