Responsibilities

• Establish nursing diagnoses with the interdisciplinary team.
• Plan and update care based on diagnoses.
• Administer medications and monitor effects.
• Perform skin assessment and wound care.
• Instruct patients and families about health concerns.
• Supervise unlicensed assistive personnel when needed.
• Document assessments and care in EMR; maintain legible records.
