Job summary

CareMed Health Solutions in Cincinnati seeks an experienced Nurse Practitioner to deliver primary and preventive care to residents in long-term care facilities. You will manage acute changes, coordinate with an interdisciplinary team, and develop care plans in collaboration with physicians.

A Ohio NP license, board certification, and DEA licensure are required, with experience in geriatric care and EMR systems.

Qualifications

• Current, unrestricted NP license in Ohio.
• Board Certification required.
• DEA licensure required.
• Clinical background in adult, family or geriatric care.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.

Responsibilities

• Provide primary and preventative care for long-term care residents.

• Prescribe medications in accordance with Ohio law.

We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities.

NP ROLE AND RESPONSIBILITIES:

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.

• Manage Residents with acute change in condition.

• Advanced care planning and chronic care management.

• Prescribe appropriate medications, including controlled substances in accordance with OH Law.

• Order, review and/or follow-up with treatments and diagnostic testing.

• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.

• and/or the care plan.

• Collaborate with primary care physician.

NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

• Current, unrestricted NP license in the state of Ohio.

• Board Certification.

• DEA licensure.

• Clinical background in adult, family or geriatric specialties.

• Experience working in a long-term care setting.

• Ability to work with an EMR.

• Ability to manage patients with advanced illness and multiple chronic conditions.
