Job summary

CareMed is seeking a Nurse Practitioner to provide primary and preventive care for residents in long-term care facilities in Ohio. You will manage acute changes, coordinate care plans, and collaborate with the interdisciplinary team to deliver high-quality care.

Requirements include an unrestricted Ohio NP license, board certification, and DEA licensure, with experience in long-term care and EMR systems. ASign-on bonus is offered.

Qualifications

• Current, unrestricted NP license in Ohio.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience in long-term care setting.
• Experience working with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.

We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities.

NP ROLE AND RESPONSIBILITIES:

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage Residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with OH Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with primary care physician.

NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

• Current, unrestricted NP license in the state of Ohio.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.
• Psych experience a plus.
