Job summary

CareMed Health Solutions is seeking a Nurse Practitioner to provide primary and preventative care to residents in long-term care facilities in Ohio. You will manage acute changes, engage in advanced care planning, and coordinate with nursing staff and primary physicians to ensure the best patient outcomes.

Qualifications include an unrestricted Ohio NP license, board certification, and DEA licensure, with experience in geriatric or adult care and a strong EMR proficiency.

Qualifications

• Ability to work with an EMR and coordinate care with teams.

Responsibilities

We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities.

NP ROLE AND RESPONSIBILITIES:

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage Residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with OH Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with primary care physician.

NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

• Current, unrestricted NP license in the state ofOhio
• Board Certification.
• DEA licensure.

• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
