Job summary

Lebanon Health and Rehabilitation Center is seeking a Physical Therapist (PT) for our facility. The PT will assess referred patients, design and manage rehab programs, and educate patients and families. Supervision of PTAs and collaboration with all disciplines are essential.

HIPAA and Residents Rights must be upheld. This is a PRN weekday position at our center. Experience in long-term/geriatric care and strong documentation skills are preferred.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Oversee and supervise Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care preferred.
• Well versed with documentation writing.
• Evaluations, weekly progress notes, recertification and discharge documentation.
• Collaborates with all disciplines to provide patient-centered care.
• Exceptional patient service skills and clinical excellence.
• Punctuality and professionalism in scheduled hours.
• Perform all other duties assigned by Director of Rehabilitation.
• HIPAA confidentiality and Residents Rights compliance.
• Experience with Case Mix Index and PPS scheduling preferred.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals per patient capacity.
• Document patient condition and educate patients and families.
• Supervise Physical Therapy Assistants and collaborate with all disciplines.
• Maintain confidentiality and ensure HIPAA compliance.
