Job summary

Lebanon Health and Rehabilitation Center is seeking a Physical Therapist (PT) to join our PRN weekday team. The PT will assess referred patients, develop and supervise rehab plans, document progress, and educate patients and families while collaborating with the care team in a long-term care setting.

The role requires licensure, experience with geriatric care, and strong documentation skills; we value professionalism and timely attendance and offer competitive compensation.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current state license and registration.
• Experience with long-term care or geriatric populations preferred.
• Experience documenting patient progress and discharge summaries.

Responsibilities

• Assess referred patients and provide direct patient care.
• Establish and administer treatment programs with goals.
• Document and record patient conditions; educate patients and families.
• Oversee Physical Therapy Assistants and collaborate with the care team.
• Maintain confidentiality and HIPAA compliance; respect residents' rights.
