Job summary

Lebanon Health and Rehabilitation Center is seeking a Physical Therapist (PT) for our facility on a PRN, Weekdays basis. The PT will assess referred patients, develop and implement treatment plans, and document patient progress under the direction of the Physician.

Responsibilities include supervising Physical Therapy Assistants, collaborating with other disciplines, ensuring confidentiality and compliance, and maintaining professional standards.

Qualifications

• Graduated from an accredited Physical Therapy college or university.
• Must hold a valid professional license and registration for state of practice.
• Overseeing and supervising Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care preferred.
• Strong documentation writing skills.
• Experience with evaluations, progress notes, recertification and discharge documentation.
• Collaborates with all disciplines to provide patient-centered care.
• Excellent patient service skills and clinical excellence.
• Punctuality, scheduled hours adherence and professionalism.
• Performs other duties as assigned by Director of Rehabilitation.
• Confidentiality, HIPAA and Residents' Rights compliance.
• Experience with Case Mix Index and PPS scheduling is a plus.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer a treatment program with goals based on patient capacity and tolerance under physician direction.
• Set realistic goals and document patient conditions; educate patients and families in physical therapy methods.
