Humana Inc. is seeking a Physician Collaborator for a fully remote role with CenterWell Senior Primary Care. You will work with APPs providing wraparound virtual services for patients with fractures, reporting to the Associate Director and averaging about 5 hours of work per month.

You will review charts, close care gaps, coordinate monthly meetings, and serve as a resource to APPs to maintain high-quality patient care across centers.

Competenze

• Must have unrestricted medical license in KY, TN, MO, NC, TX, IN.
• Graduate of MD/DO program of an accredited university.
• Board certification or Board Eligible.
• 2+ years of experience collaborating with mid-levels.

As a Physician Collaborator you will share our vision to improve health care in the communities we serve! This is a fully remote role, with an average of 5 hours of work monthly. You will report to our Associate Director.

You will collaborate with our APPs who provide virtual wraparound services to support patients who have had fractures in the last 6 months when they cannot get in with their own PCP.

Required Qualifications

• Must have unrestricted medical license in the following states: KY, TN, MO, NC, TX, IN
• Graduate of MD/DO program of accredited university
• Board certification or Board Eligible
• 2+ years of experience collaborating with mid-levels

Work at Home Requirements: To ensure Home or Hybrid Home/Office employees’ ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria: At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested. In certain roles, the minimum recommended internet speed required by Humana may not be sufficient for business needs. Humana reserves the right to require associates to upgrade their internet service if necessary. Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.

Travel: While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$310,000 - $368,500 per year
