Job summary

Harbor Hospice is seeking a Spiritual Care Counselor/Chaplain for PRN assignments in Muskegon, Michigan. You will provide compassionate spiritual care to patients, families, and caregivers, based on assessments and patient preferences.

You will coordinate with the care team and community clergy to support faith-based needs and memorial services when requested. Required qualifications include a seminary degree (BD, M.Div.

Qualifications

• Graduate of accredited seminary school of theology (B.D., M.Div. or equivalent).
• Clinical Pastoral Certification (CPE).
• Experience working with death and dying, patients/families/caregivers.

Responsibilities

• Provide spiritual care to patients, families, and caregivers.
• Plan and carry out interventions based on spiritual assessments.
• Counsel patients and families regarding spiritual distress and needs.
• Document ongoing communication with IDT and community clergy as needed.
• Assist in memorial services and supervision of spiritual care volunteers.

Spiritual Care Counselor/Chaplain - PRN StatusSUMMARY OF JOB RESPONSIBILITIESThe Spiritual Care Counselor is responsible for the provision of spiritual care services to patients/families/caregivers of Harbor Hospice.Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.ESSENTIAL FUNCTIONS AND DUTIES (other duties may be assigned)Maintains the confidentiality of patient and employee information at all times, both at work and off duty, and after the termination of employment. According to the agencies Notice of Privacy Practices and HIPAA regulations.Is knowledgeable and functional in computer applications including database, word processing, spreadsheet, and e-mail system as required for the execution of the job.Knowledge of Hospice concept and philosophy.Plans, develops and carries out interventions based on the spiritual assessment.Counsels patient/family/caregiver as needed in relationship to spiritual distress, support issues and other identified spiritual difficulties according to their traditions and preferences.Evaluates the effectiveness of planned interventions and revises plans of care accordingly.Connects patient/family with their faith community as needed.Documents ongoing communication with community clergy as needed.Provides consultation, education, and support to the IDT on spiritual care upon request.Demonstrates open communication style with patients/families.Assists in periodic memorial services to meet the needs of staff, volunteers, and the community as assigned.Assists in supervision of spiritual care volunteers and visiting patients/families/caregivers.Follows standard precautions/infection control policies.QUALIFICATIONSGraduate of accredited seminary school of theology (B.D., M.Div. or equivalent theological degree).Clinical Pastoral Certificate CPEExperience working with death and dying, individuals/family/caregiver.Current Drivers License.
