Job summary

Valor Hospice in Dallas, TX is seeking a Chaplain to provide spiritual care in the home for terminally ill patients and their families. You will assess spiritual needs, provide counseling, and help coordinate support from local clergy or pastoral counselors.

You will participate as a core member of the Interdisciplinary Group/Team (IDG/IDT) and help plan patient care. The role requires a Master of Divinity preferred and at least one year of hospice chaplain experience.

Qualifications

• Master of Divinity preferred.
• Minimum one year of chaplain experience in healthcare preferred.
• Strong interpersonal and communication skills.
• Knowledge of community spiritual resources and networks.
• Valid driver’s license and reliable transportation for field visits.

Responsibilities

• Assess patients and families to identify spiritual needs and document in care plans.
• Provide pastoral counseling to meet identified spiritual needs.

Job description

Primary function is to provide spiritual care in the home to terminally ill patients and their families. To provide anassessment of the patient’s and family’s spiritual needs and to provide counseling to meet those needs. Makes reasonable efforts tofacilitate visits by local clergy, pastoral counselors or other individuals who can support the patient’s spiritual needs. Participates asa core member of the Interdisciplinary Group/Team (IDG/IDT).Essential Functions:Assess the Hospice patient and support system to identify spiritual needs as evidenced by the Plan of Care (POC),documentation in the clinical records, and IDG/IDT conference minutes. Provide support for the spiritual needs ofpatient/family.Attend IDG/IDT meetings and participate in the patient care planning process. Participate in Quality AssessmentPerformance Improvement process as required.Use effective interpersonal relations and communication skills, facilitates the use of these skills by other team membersto achieve desirable outcomes.Meets mandatory continuing education requirements of the Hospice.Demonstrate commitment, professional growth, and competency by maintaining a working knowledge of public andprivate eligibility standards and requirements.Promotes the Hospice’s philosophy and administrative policies to ensure quality of care.Demonstrates understanding of coordination and continuity of care by collaborating with the IDG/IDT.Accept clinical assignments that are consistent with education and competence to meet the needs of thepatients/families.Education: Master of Divinity preferred.Licensure: Not Applicable.Experience: One year of experience as a Chaplain in a healthcare institution preferred.Skills: Working knowledge of community spiritual and other resources.
