Job summary

Foothill Acres Rehab and Nursing Home in Hillsborough, NJ is seeking a Speech Therapist for a Per Diem position at 12 to 15 hours per week. The role may require additional hours to cover overflow, vacations, sick time and holidays.

Flexibility to work weekdays and weekends is preferred. The therapist will assess and treat patients, develop and implement treatment plans, and collaborate with the care team to optimize communication, cognition and swallowing skills.

Responsibilities

• Optimizes communication, cognitive and swallowing skills of patients by performing or supervising therapy staff in initiating referrals and evaluating and treating appropriate patients.
• Establishes individual patient schedules with other team members.
• Develops treatment plans with problem lists, long- and short-term goals, and methods to achieve them.
• Coordinates treatment plan with patient, family and other team members; implements and monitors progress.
• Documents results of assessment, treatment, follow-up and termination of services.
• Recommends and obtains equipment to improve patient communication; instructs on augmentative communication devices.
• Instructs patient, family and staff on speech language procedures or home programs for carryover.
• Reviews quality and appropriateness of speech language pathology services using predefined criteria.
• Submits billing, payroll and expenses accurately and timely; assists in collecting statistics.

Per Diem position
Foothill Acres Rehab and Nursing Home in Hillsborough , NJ, is looking for a Speech Therapist to work on a Per Diem basis for 12 to15 hours per week. We are looking for an SLP with additional hours to cover overflow, vacations, sick time and holidays. • Optimizes communication, cognitive and swallowing skills of patients by performing or supervising therapy staff in initiating referrals; evaluating and treating appropriate patients; effectively participating in the care planning process; appropriately documenting progress toward identified objectives and communicating patient status and needs to the patient, the patient's family, the staff and other professionals as appropriate.
• Establishes individual patient schedules in conjunction with other team members.
• Develops treatment plans by identifying a problem list, including long and short-term goals and methods to achieve identified goals. Confers formally and informally with the physician and other team members in coordinating the total rehabilitation program of the patient.
• Coordinates treatment plan with patient, family and other team members. Implements and conducts treatment as outlined in the treatment plan. Monitors patient's response to intervention and modifies treatment as indicated to attain goals in accordance with physician's orders. Adapts treatment to meet the individual needs of the patient.
• Documents results of patient's assessment, treatment, follow-up and termination of services.

• Instructs patient, family and staff in appropriate speech language procedures or home program to allow carryover of therapeutic remediation and goals in daily activities.

• Assumes responsibility for ongoing continuing education and professional development. Attends in-service training as well as provides education to members of the patient care team and members of the community.
