Job summary

Louisville East Post Acute is seeking a licensed Speech-Language Therapist to provide PRN services across our facility, including Pathways TBI, Sub Acute Rehab and LTC units.

You will evaluate and treat residents, communicate with families and physicians, and maintain documentation in the medical records as directed by the Director of Therapy to ensure quality care.

Qualifications

• Licensed Speech-Language Pathologist with current credentials.
• Ability to evaluate and treat residents across units.
• Accurate documentation and communication with families and physicians.

Responsibilities

• Evaluate and treat Residents in Pathways TBI, Sub Acute Rehab and LTC Units.
• Communicate with Families, Physicians and health team members.
• Maintain accurate documentation in medical records per Director of Therapy.
