Job summary

Sagewood in Phoenix, AZ is seeking a PRN Speech Language Therapist to provide inpatient services in Med A settings and collaborate with an amazing in-house team dedicated to enriching the lives of seniors through compassionate care and innovative programs. You will evaluate and treat residents, maintain records, report billing information, coordinate with staff, and support families.

A Master;s in Speech Language Pathology, CPR certification, and state licensure are required.

Qualifications

• Masters degree in Speech-Language Pathology required.
• CPR certification required.
• State licensure in the designated geographic area required.

Responsibilities

• Maintain and ensure accuracy of patient care records for all patients on personal caseload.
• Report billing information for Speech Therapy for all patients on caseload.
• Provide direct patient care including evaluation and treatment planning.
• Report and interpret treatment results.
• Perform screenings, evaluations, and develop/deliver treatment plans per policies.
• Participate in patient care meetings as appropriate.
• Maintain a high standard of resident care and educate families and staff on treatment plans.

We are seeking a true PRN team member

who can provide coverage during vacations, sick calls, and other staffing needs as they arise. Join an amazing in-house team dedicated to enriching the lives of seniors through compassionate care and innovative programs. As a Speech Language Therapist, you’ll provide exceptional services within Assisted Living, Skilled Nursing, and Memory Care residents. This role will foster a positive, supportive environment for residents, families, and team members on a PRN basis.

PAY RATE: $42 - $60/ hr

What you'll do:

• Responsible for the maintenance and accuracy of records regarding patient care for all patients on personal case load.


• Responsible for the correct reporting of billing information of Speech Therapy regarding all patients on personal case load.


• Provide direct patient care as prescribed, including evaluation and treatment planning.


• Responsible for the reporting and interpretation of treatment results.


• Perform clinical functions: screens, evaluations, development of treatment plans, and delivery of treatment in accordance with facility, Medicare, and Therapy Management Corporation’s policies and procedures.


• Participate in meetings regarding patient care as appropriate.


• Maintains a high standard of resident care.


• Provide education to patient’s family as well as staff on the specific treatment plan as well as therapy goals for the patient.


• Responsible for the coordination and cooperation with other staff members in carrying out the treatment plan as established.


• This role primarily works in inpatient Med A.

Who you are:
