City Creek Post Acute Care in Salt Lake City, UT, is seeking a PRN Speech-Language Pathologist to join our in-house rehab program. Screen and evaluate patients with communication, cognitive, or swallowing disorders and develop care plans, with emphasis on regulatory compliance.

You will deliver rehabilitative treatment, evaluate outcomes, provide counseling to patients and families, and generate discharge summaries.

Qualifications

• Requires SLP license and eligibility to work in Utah.
• Open to SLPs at all experience levels.
• Location: Salt Lake City, UT.

Responsibilities

• Screen and evaluate patients with communication, cognitive, or swallowing disorders and develop care plans.
• Deliver rehabilitative treatment while ensuring regulatory and clinical standards.
• Evaluate outcomes and modify services; make referrals as needed.
• Provide consultation to patients, families, and other providers.
• Generate discharge summaries per regulatory requirements.
