Job summary

Avamere Transitional Care at Sunnyside in Salem, OR is seeking a Speech-Language Pathologist (SLP) to provide and direct all aspects of patient care, including evaluation, treatment planning, and family education, in accordance with licensure and organization guidelines.

This PRN role requires supervising assistants, documenting progress, and collaborating with the rehab team to ensure safety and high-quality outcomes.

Qualifications

• Graduate from an approved Speech Language Pathology curriculum.
• Active licensure and ability to practice.
• CPR/BLS certification required.
• Knowledge of Medicare and payer sources.

Responsibilities

• Provide speech pathology services per licensure.
• Direct and supervise assistants and rehab aides.
• Follow safety procedures for patient protection.
• Develop treatment plans and educate staff.
• Complete documentation of evaluations and progress.
• Collaborate with rehab team for quality care.
