Job summary

Garden Spring Rehab and Care Center in Willow Grove, PA is seeking a PRN Speech Language Pathologist to join our rehab team in providing excellent patient care.

The role involves assessments, direct patient care, and collaboration with the rehab team to develop and implement treatment plans for residents in a long-term care setting. PA licensure is required; prior geriatric care experience is preferred.

Qualifications

• Must have graduated from an accredited Speech Therapist college or university.
• Must hold a valid professional license and PA state registration.
• Experience with long-term care and/or geriatric care is preferred but not required.

Responsibilities

• Assess referred patients requiring rehabilitation services.
• Provide direct patient care and document condition and progress.
• Establish and administer treatment plans with goals.
• Record weekly progress notes, recertifications and discharge documents.
• Educate patients and families on therapy methods.
• Collaborate with other disciplines to ensure patient-centered care.
• Perform duties assigned by the Director of Rehabilitation.
• Maintain confidentiality and comply with HIPAA and Residents' Rights.
